Form for songs recorded without permission

As I have mentioned on this previous post, I had many songs recorded without my permission. 

Some of them became very famous. 

If you recorded any of my songs, you must contact me via this blog to regularize your situation (read previous post). I have put together a form with most common issues regarding song use for each song.

For artists/bands that used my audiovisual material, you must sign a separated contract from the songs for specifying author credits.
